This is NOT an order form. This is a registration form solely for signing up as Usana member. Those who wants to order the products without signing as member, please kindly head to “Email Us” page and leave your message there.

By filling up this form and submitting it to me, you are agree to be enrol as Usana member and RM99 membership fee will be charged to your credit/debit card.
Credit/debit card details need to be provided for your online ordering, enrollment or membership renewal. [To those who doesn't has both, you may leave it blank]. I am willing to lend my card but for one time use only [you need to transfer the amount to my account then]
Bank account details need to be provided for Usana to transfer your weekly bonus [if applicable]
Shipping fee is RM4 for any amount of order. However, to deliver the member kit only [without placing order for any other Usana products] the shipping fee would be RM10.
